Maybe a tired young lawyer is hunting down a disillusioned ex doctor and bringing him tp justice while he has to work with his own fked up childhood and his constant pragmatic ways of solving issues, wanting to conform to ideals that don't really help him. While also dealing with his estranged sister om the loose running after him.

Make it worse, by having the ex doctor be the reason our hero had so much hope in goodness amd fairness. Now seeing his idol stray from his path, he doesn't know what to do with his own morality.

This actually sounds. So amazing. In fact, I already have so many angsty, heartbreaking, and grotesque ideas forming for this reverse au you wrote here. I can see it now.

I can imagine Dr. Tenma's downfall rooting from Eva calling off their engagement, and Udo Heinemann demoting him after rebelling his orders to not perform surgery on young Johan (instead of him running from his career in order to take down the Monster he supposedly brought back to life). It might not sound like a plausible vengeful origin story. But let's face it. In this case, when trying to grow as a person, and creating the perfect life, only to be thrown back down out of nowhere with absolutely no one to lean on—can destroy a person. Because whether we'd like to admit it or not, if it weren't for Johan granting Tenma his wish, and those doctors mysteriously dying, Tenma's life wouldn't have improved like it did (example: being promoted to Chief of Surgery.) Though, even then, his reputation was still at stake when Lunge got involved, and assumed Dr. Tenma was the culprit after witnessing how much the doctor benefited from his peers being murdered.

Moving on to Johan, now. Like he said, himself—he saw Dr. Tenma as a father figure the moment he was saved from a terrible fate that would have taken him at a young age. Which he's never had anyone go as far as to RISK themselves for him. All of the adults in both Johan's, and Anna's life have been cruel, and proved over again that no one could be trustworthy. Until Tenma refuted that idea. It left Johan wanting nothing more but to better himself, and learn from his trauma instead of using it to deceive, or harm anyone around him. As he becomes a young adult, his mind and body actually allow him to experience, and show true emotions. He no longer lets the past consume him. "Tomorrow will be a better day." He becomes a law student, is actually able to build genuine relationships and bonds, and also has the goal to reunite with both his beloved sister he was separated from after the Liebert's incident, and the doctor that he considered to be the father he never had.

Though, he's torn coming to find out that the doctor is not who he used to be. Tenma is working underground, as a black market doctor. But not like the one we saw in the manga/anime, or hear about in the sequel, Another Monster. He's one who'd conduct evil, and inhumane experiments. Because "not everyone is born equal," as Eva had once told him, and he uses that to his advantage. His healing hands are used to make everyone who will ever doubt, or walk all over him—regret. It's a power rush he gets after being wrongfully used at the Eisler Memorial Hospital.

Johan struggles. Battling his morality while now hunting down the doctor he once looked up to, and attempting to reconcile with his sister who will possibly never forget what he put her through, or ever forgive him for it, too. What will he do?

The Great?? You know this means we need a real world AU, arranged political marriage, enemies to lovers horny hours (even though it's late) The Targaryens are the perfect parallel to old european ruling dynasty. Marrying cousins, nieces and nephews, uncles and aunts, all to consolidate their power. Cause Aemond being a king or emperor and his court and expectations demanding he take a bride and he'll ONLY ACCEPT someone of valyrian blood? only there's only ONE non betrothed valyrian woman in the whole of europe and that's Valaena. aemond maybe seized power after his brother aegon was unfit to rule, and while he is an excellent emperor, he is trying to shore up his power and support, and the best way to do that is through an advantageous marriage and expeditious creation of heirs

he and his advisors start searching, but aemond streses the importance of valyrian blood, of emphasizing his divine right to rule through the power of their blood, and his heir needs it too

enter valaena, whose parents are the other most powerful rulers on the continent,,, and while as the only daughter of rhaenyra targaryen, the interest in her is ASTOUNDING, she's managed to drive off every other suitor (it doesn't help that daemon and jace and luke stand there menacingly supporting her antics everytime she alienates a potential suitor bc theyre "not good enough for her")

rhaenyra is near the end of her wits, because she HAS to marry valaena off and rhaena and baela were none of this trouble for laena when she got them married off too (though to be fair, rhaena married luke and that's a win win),,, enter aemond's offer

aemond makes an obscene offer, needing the support of rhaenyra's kingdom AND the legitimacy a valyrian bride will offer him,,, but he's expecting a wife to a be a nuisance, to get her pregnant and find pleasure elsewhere, for some timid little thing to be delivered to him bc that's what the ladies of his mother's court are like

desperate, rhaenyra accepts it against valaena's wishes, against daemon's wishes, because at the very least she knows what her brother is like, and val will be protected in the coming chaos

so she's shipped to aemond , fighting tooth and nail to keep composure and do her "duty" when she's screaming inside, tortured by the fact that she avoided marriage for so long just to end up wed to her mother's imposing long estranged brother

the first time she meets aemond, she's struck by her attraction to him, to the way he easily commands her, to the way it scares her how much he seems to want her (and to control her)(and how much she wants him to control her)

she may smile, she may grit her teeth and say her vows, but she vows to herself that she will never allow herself to be just a consort, just a wife, just another pawn in aemond's politcal machinations

except aemond is determined to get an heir on her, duty warping into breeding kink, because he does indeed take care of her,,, he gives her everything she didn't know she needed in the marriage bed

and valaena is initially incredibly unpopular at court, quarrelsome and unwilling to put on "airs", just her normal blunt self, eschewing convention and expectations, unapologetically seizing power until she truly is aemond's equal

and aemond loves every second, completely obsessed with teh woman he just expected to tolerate, adoring his little spitfire dragon wife who only seems to relax when she's covered in bruises and jewels with his cum dripping out of her, giving in to her every whim

and gods help them when she does get pregnant

Sometimes you just have one of those moments where the progress we've made as a culture get thrown into stark relief. You look at something and go "Holy shit, that would never have happened when I was a kid."

Today, I had one of those moments when I realized that the teenage boys I'm working with are just. genuinely, openly enthusiastic about going to Build-a-Bear for their outing.

These are sixteen and seventeen year old boys! They just had a whole conversation about what to name their "cute", mostly new squishmallows! They're genuinely excited that they're going to Build-a-Bear this weekend and asking other kids to pick up specific accessories for them!!

Holy shit, that never would've happened when I was 16. None of the boys would have dared to be visibly interested - and neither would most of the girls! There would have been a million gay jokes and "Haha, you're a girl" jokes and "What are you, a baby?" jokes. Teenagers weren't even supposed to care about anything back then!

Less than 15 years later, and I'm watching three 17 year old boys treat all that as not even worthy of comment.

So let's call that a reason for hope. Even when the kids aren't alright, in some ways apparently they are alright. Go Gen Z, honestly. It's so lovely to watch you guys just openly doing and saying stuff that, when I was a teen, would've been a social death sentence.
